Trip Forms & Documents
If you are interested in a trip, and the trip is over 6 months away, please fill out the “Intent to Travel” form. This will put your name on that particular trip contact list so you will be informed as to any information concerning that trip.
After sending in the “Intent to Travel” form, please begin to fill out the Application and Emergency/Contact form and send it in as soon as possible.
If the trip is less than 6 months away, you will need to submit the Emergency/Contact form and the “Application.” Upon receipt of the Emergency/Contact form your spot on the trip will be reserved. The Application should follow the Emergency/Contact form as soon as possible.
Note: The forms are to be printed out and sent via mail.
Another Note: The Emergency/Contact form needs to be notarized.
